Yes. That is the teaching. And that is why it is important to try to live in a state of grace.
Even if you have lived and otherwise righteous life, dying with an unconfessed mortal sin puts your soul in danger....of course we also believe in a merciful God...It is ultimately up to Him to determine where the condition of our soul dictates for our destination.

Yes. A person who dies in mortal sin cannot enter the kingdom of heaven and is doomed to eternal suffering in hell.
