It acts like it will quit on me, but only when idle, and after its been driven for at least 15 minutes.

Just wanted to know all the possible options I'm looking at when getting it fixed.

if the problem is only at idle and when warm I would suspect that the EGR valve is hanging open. The Exhaust Gas Recirculation valve is designed to operate when the engine is warm and when the engine speed is over 2000 rpm. if the valve is clogged with carbon it would cause too much air (relative to the amount of fuel that is injected) to get into the engine at idle and cause the car to shake and feel as though it will stall out. If you are sitting at a stop and experiencing this, put the car in neutral and raise the engine speed to about 2000. if the problem goes away and the engine smooths out, you can suspect the EGR as that is the point where the valve should open and everything should be in balance.

there is also the possibility of a vacuum leak in the engine but vacuum leaks would affect engine performance at all times, not just when warm.

if the idle is bouncing up and down, its air idle control valve. the other guy with egr and vacuum knows what he is talking about. however vacuum leak would not be noticeable while driving unless it was a huge leak.
