They can start telling as soon as the 2nd month, but you may still have a chance for twins after even if they only saw one 'yolk sack'. When doing the first ultrasound to check for the size of the baby they normally check to see if there is more than one sack as well, even if its not likely that you can have twins.

At your first ultrasound they will look into your uterus and check all quadrants to see if there are multiple sacs. They should be able to tell at 16-20 weeks easily. Anything before that is a little hazey and things get left out.

I would think they could tell by your blood test with how high your hcg levels are. When my sister did IVF they knew if more than one embryo had implanted by her levels. If not by that then with an ultrasound.
