The difficulty in any type of partner dancing is less the steps, but more the paired co-ordination. Anyone can learn a routine, which is what you see on "Dancing with the Stars", but to go to a club and dance with others, you have to know numerous steps and understand (male) how to signal the lead to the lady, (female) how to follow the man's lead signal. There are standard lead techniques, but many of them are very slightly different from one another, and each man performs these leads differently depending on training and level of confidence. Take heart, salsa is a fun dance that can be easily learned (at least the basics) and the way it is danced in the clubs is very different and much looser than in the ballroom. A lot of clubs, bars and dance schools offer pay as you go dance classes to fill up slow days/nights. Take a few lessons and get the feel. Relax and enjoy. Then hit a club or group dance, find a strong/skilled partner, and let them know you are a beginner, so be kind. While some dancers will decline to dance with you, others will be more than happy, helpful and fun. The main secret for women is to learn the basic salsa step and do it constantly, to the rhythm, no matter which way a guy pushes, pulls, twirls or slides you.

I'd say, the most difficult part about salsa dancing is dealing with sleazy guys. I mean, most people are nice, like 99% of them, but you inevitably run into one of those...
I'm just joking. Different people have difficulties with different things. I don't know what step would i call the most difficult - they are all kind of the same - you watch your teacher and you learn, but this looks rather fancy: http://youtube.com/watch?v=ELLB93oABrU&mode=related&search=
Try ballroom lessons at your local studio. They most likely etac salsa as well, but they are likely to be doing a better job than nightclubs. Check out http://www.ballroomdancers.com

Patterns are just patterns. The steps hardly matter at all. Technique and connection are what counts. I do a little ballroom dancing, so on those rare occasions when I go to a salsa club I invariably have somebody ask if I'm an instructor, just because anything like decent technique is so uncommon, and I'm not impressed by "flash and trash," so it certainly isn't because I'm doing anything fancy.
