The best way is through repetition of homework problems. There are only so many different kinds of problems that the teacher can assign. Learn all the different types and practice, practice, practice. This way you won't be thrown off on the test by a problem you have not seen before. Good luck.

You have to keep studying. Do homework, go to tutorials, ask for the correct answers from your teacher, and in general, show that you care. Even if you are not a GENIOUS, showing that you care will get an apprectiation from your teacher.
To start getting better grades, you have to keep practicing like crazy. I admit it will most likely not be fun, but practice will get you a long way. And once you spend a week or two studying like crazy, the rest will be much easier, because math is a subject that builds up on stuff that you already know.
To fix any grades, try talking to your teacher about re-doing assigments, or doing extra credit.
Either way, you'll start showing that you actually care about the subject (even if you don't, because I understand that math is one of those things that sucks for a lot of people =]). Start getting a better relationship with your teacher, so that he or she can help you with studying and notice that you care.
Effort is definitely a big part of math, and a lot of the time a big part of your grade. Remember that!

If you don't already, do your homework. You can study all you want, but you need practice.
If that doesn't do it, go back over all of the problems you miss (on homework, quizzes, tests, etc.) and point out what you did wrong...then find the right answer. Is there a common mistake that you could try to avoid?
If you still can't raise your grade, do extra practice (from your textbook or workbooks or even from a library book). Then ask your teacher for the answers to check your own answers. Review what you got wrong and get your teacher to help with those, too.
If all else fails, do any extra credit you can...I can't think of anything else to raise your grade.

Hope that helps.

I will suppose you are in middle school or early high school.
Put your calculator away and go back to basic arithmetic by memorizing the multiplication tables and the squares of numbers through twelve. Practice. Practice. Practice. Quit watching TV. Turn off your music. This will increase your speed. First look-over the whole assignment, be it classwork or test. Then always do the easy questions first, then the hard ones. This will increase your speed. The calculator will help you with the more complicated problems and numbers which you cannot resolve by inspection. Do not use it any other time. This will increase your speed.
Doing complicated math problems is like eating an elephant. You just do it one bite at a time.
Be neat , write big and use all the paper you need. Paper is cheap.
When doing "word" problems, the equal sign often goes where the verb "is" is.

Yeah, I have the same problem sometimes. I think you should probably ask the teacher if you can re-take some of the quizzes that are lower than a C. Also, try practicing math problems, even if you think you know how to do them. Make sure to double or triple check your answers. Simple mistakes can really kill your grade (I should know, I lose a ton of points to simple mistakes). It might help to get a tudor on problems that you really don't understand. Hope it helps!

Relax
Do your homework
practice practice and it will come second nature
use your library to practice the math problems from other books
writing it out is better than reading it because you are performing the action and experience how to do something is the best teacher of all

the best way to improve your grade (in my eyes) is

not to continuously look at a text book,

doing maths problems is actually better for revision.
in maths you hardly take any notes.

get the gist of a topic and apply it to lots and lots of questions,
(get you teacher to help as well)
you should over time understand why you use the equations and stuff.

Ask her for extra credit, look for mathematical patterns in music or random math throughout culture like the tax rate is 8% so the $1 menu is actually the $1.08 menu, etc

Do practice problems. Check your answers AND your work. And find out where you keep getting messed up so you know where you need to be more careful.

Keep asking your teacher questions. You have to show him/her that you really want to learn and are trying.

to raise your math grade, do not read...keep solving, write, write your formulas, dont read....keep practising.... nothing better than that....randomly ask yourself formulas....this habit will help u for the rest of ur life.

see your book, try to solve the already solved problems without looking, then compare your answer, and see where u tend to go wrong / confused.
