A survey, only because you asked for elaboration. I think a poll would indicate the need for a simple yes or no. A survey conveys ideas a little deeper than a poll would. IMHO.

So, it can be a poll or a survey.
Your popularity will be reflected by the number of stars and answers you get.
In fact you are already asking Y or N to whether it is a POLL or SURVEY.

It's a survey because your asking to see what it is, a poll would have been people picking one off many options.

It would be a survey. In polls you would be choosing an answer from provided answers. In surveys you are collecting "data" on groups of people.
And atheist. And no I don't want to be saved.
