I give injections all the time, and the only time I have any trouble with medication NOT coming out of the needle is if the needle isn't screwed onto the syringe tight enough.
Sometimes you hear a sputtering sound around the connection, and not much liquid comes out of the needle. Usually the nurse finds this out when she expels excess air out of the syringe PRIOR to inserting the needle into the patient. It is not generally accepted good nursing practice to insert a needle twice for ANY reason. And you would NEVER EVER EVER EVER cut the tip off a needle. The rule is, if it doesn't work, throw it away and get one that does! However, as long as the site was properly cleaned, I doubt you'll have any real problems with it. If you experience any unusual redness or irritation, talk to your health care provider.

Sometimes the single injection set doesn't work well and requires another attempt. That should be done with a new needle. As long as you're the same person who recieved the first injection that the Depo didn't come out completely, it's Ok to get a second injection for the rest of the meds. Bad news but it happens. Could also be limited skill or knowledge on the nurses part, but these things do happen from time to time.

Sounds like the person didn't know how to change out the needle...she should have just switched the needle and then reinserted the injection...I dont think they are suppose to use it twice...i would tell the doctors office what she did...hope this was some help...good luck...
