Four strikeout innings have happened several times.
Chuck Finley has done it more than once (May 12, 1999 August 15, 1999 and April 16, 2000).
There's never been a 5 strikeout inning in the major leagues.

Considering a full inning contains six putouts, sure, it's happened somewhere in the past.
But you probably mean a single, offensive half-inning. The major league record is four strikeouts. The minor league record is five; the last time I know of it happening was in the early 90s by Kelly Wunsch. Knuckleballer, he; well-established as the pitcher type of choice to get those dropped third strikes often.

It's happened plenty of times, as has been mentioned.
Kerry Wood had a shot at 5 K's in an inning in 2002. Two runners got on base after a third strike was not caught, however he got one out on a ball put in play (I think it was a ground out).

Randy Johnson did it to the Dodgers in 2003. He struck the first two hitter out, then threw a pitch for strike 3 that went by the catcher, then Johnson struck out the next time in 3 pitches.
That's the only one I ever saw on tv.
