I've heard it was 4, but i'm not sure.

If someone knew who did it, when, and how many that'd be great.

The most in an 9 inning game is indeed four, and quite a number of players have done it.

For the AL
Rocky Colavito played for Cleveland and did it on 06-10-1959
Mike Cameron played for Seattle and did it on 05-02-2002
Carlos Delgado played for Toronto and did it on 09-25-2003
Lou Gehrig played for the Yankees and did it on 06-03-1932

For the NL
Joe Adcock did it for Milwaukee on 07-31-1954
Ed Delahanty was on Philadelphia and did on 07-13-1896
Shawn Green played for Los Angeles and did it on 5-23-2002
Gil Hodges played for Brooklyn and did it on 08-31-1950
Bob Horner played for Atlanta and did it on 07-06-1986
Bobby Lowe did it for Boston and was done on 05-30-1894
Willie Mays did it for San Francisco and did it on 4-30-1961
Mark Whiten did it for St. Louis and did it on 09-07-1993.

Player: four, done 15 times or so. Most recent was Delgado in 2003. (See link for complete list.)

Team: 10, by the Blue Jays against the Orioles.

Both teams: 12, done twice, both times a Tigers-White Sox game.
