At least in the case of Butch Cassidy, it was because he was a butcher for a short amount of time. Typically butchers would be called Butch. Had nothing to do with Cassidys real name of Leroy Parker but its a good sounding alias.

I've heard butch used for any number of guys names but have also heard it used in my town today for a man who was a butcher and his son got stuck with that name also.

It can be a generic nickname, but I've heard it mostly being given to a Richard or Scott.
