go to guest services and explain your situation.
As others have noted, some parks do require you to document your disability so if you are uncomfortable, you may want to take a different route

You will go to Either City Hall inside Disneyland, or Guest Relations inside of California Adventure. Let them know what your concern/need is and they will give you a pass if needed.
I may be incorrect, but I do not beleive that you could get one for just being diabetic. You would have to have some other conditions.

Once you get to Disneyland, you can go to City Hall to get a Special Assistance Pass. I'm not sure you can get one just for being diabetic, but if you already called Disney and asked them, and they said it was ok then I guess you can.

I believe you can ask for it at the City Hall, which is right inside the front gate. I don't know the specifics, but I assume that if you ever have health problems, you can show the pass to an employee and they will help you out. If your diabetes is severe enough that it is difficult to wait in line, then you could skip to the front of the line.

you can get the pass with varouris stamps. but you must have a valid reasons you may also might need a doctors note also now.
the best way to to rent a whellchair.
the top reason to why they would give this to you
1 if you had a child that was austic. and have servere problem standing line and etc etc etc
2 if you had mobility issues like vertigo and you visting the park alone
3 sun problems and you need to be in the constanst shade at all times

the guest assistance card is nice but be preapared to be hulimated when they ask you very, very, very .personal questions. if they ask you questions ask to do it in private so other guests wont over hear what is going on and so you wont be so humilated either
cast memebers are alowed now to anser very personal questions regarding you medical condition.
