Keep playing until there are 3 outs, then start the second inning. There is no time limit or mercy rule in Major League Baseball.

There have been 1 or 2 instances of 20+ run innings over the years.

There's never been a 20+ run inning in MLB, the record is 17 (1953 Red Sox IIRC), but yes, the defensive team simply must stop the bleeding somehow.

They would have to play it out. It's not like little league where there's a mercy rule. The game could be 50-0 and they had to keep on going.

They would continue to play until the next morning, at which point they would have an MLB meeting to end the game based on the mercy rule.
