sounds like (assuming there are no leaks and coolant is full) you might have a blown head gasket

Most "newer" vehicle cooling systems are sealed systems and will have a bleeder valve. Usually located near or on the thermostat housing. And just like bleeding brakes that have air in the lines, you may need to bleed the air out of the cooling system. When air is in the system, it creates a pocket and this will cause some over-heating issues. Pressure checking the cooling system is also something that I would recommend. You may have a leak that is not easily noticed and this too could cause your problem. There are other things to check, but this gives you a start. Good luck.

12 years is old for a radiator. It's probably half clogged and the fins are separating from the tubes. Flushing it out real well will probably just cause it to start leaking. Replace that puppy.

check the coolant level is good, and perhaps change your coolant and flush the system through at the same time, make sure the pipes are clean ect . . .

if this dosent solve it i say its possible your radiator is blocked so try clearing it out.

Depending on whether or not the coolant is good, it might quite possibly be a radiator.

Also, did you bleed the air after filling coolant?
