A 1/4" wood dowel is definitely the easiest method of preventing the cake from toppling, but keep in mind you will need to cut it to the appropriate length. Make it about an inch shorter than the height of your finished diaper cake so that it doesn't show at the top.
If you want to use the ribbon method, start with your bottom layer. Remove two diapers from opposite sides of the cake, near the middle but not on it. Thread a long piece of ribbon through like you would a button, then replace the diapers. Take two diapers from the same location out of your next layer, place on top of the bottom layer, thread the ribbon through again, then replace the diapers. Keep doing this until you get to the top. Tie a big bow on top with the excess ribbon, or tie a knot, trim the ends, and tuck between the diapers so the ends don't show.
In my experience, the dowel rod is faster and easier, and makes for a very secure cake, as long as your layers are nice and tight, which is key. The ribbon method is a little trickier, and you don't want anyone to think they can pick the cake up by the ribbon at the top, that isn't what it's there for.

Well I don't make them professionally, although I would like to, but you place a baby bottle in the middle. If you are making more then one layer, you put the bottles end to end. I wrap the diapers around one another, so when you have the first layer you have plenty of room for the next layer to go on top.

I make them professionally (http://www.diapercakesbybecca.com ) and I suggest that you place a tall bottle (straight one) in the middle or tall bath products (oversized Johnson's products). You could even use a blanket rolled into a tube or RIBBON going up through the middle!!
You can also use dowel rods which can be purchased at any craft store or even Wal-Mart!!!
If your cake is PACKED with diapers you don't need a while lot of interior support because the weight of the tiers will keep the cake from toppling!!

I used a thick wooden dowel (stick) that you can buy at home depot, walmart, or any crafts stores. I then secured the area around it with rolled up wash cloths and burp cloths instead of simply more diapers. It was a pleasant surprise for the expectant mom.
