It's really not necessary to worry about this much, as ALL expenses of a decedent would be while he was still alive. Anything after death would be an expense of the estate.

However, "inter vivos" would suffice, and if you wanted to call attention to the fact that the expenses were medical expenses relating to the time when death was imminent, perhaps "pre-mortem" would be useful.
