Well, yes and no. If the burlap is 100% organic, and if the soil is moist, not a desert droughty soil, the burlap decomposes quickly. In desert soils where drip irrigation only moistens limited areas, the burlap takes too long to decompose as moisture doesn't get that far down.

If the burlap has any polyester or other inorganic material, by all means remove it.

Far more important is to remove any ties up at the top that could strangle the plant. Also if there was a wire cage around the burlap ball to hold it together, it should have been snipped away.

The problem with wholesale removing the burlap is if the root ball was not firm to start with. The burlap held everything together and unwrapping it could cause the whole root ball to collapse.

If you are confident the soil will be moist the full depth of the root ball, then go ahead and slash the burlap in several places, pull back the top (also checking for any hidden cords that could strangle the plant) and add soil. Within a year the burlap should be just about gone.

yes you do. It is only there to protect the roots and keep in moisture. When digging a hole to plant, make it twice as wide as the spread of the roots. put some onf the dirt you took out into the botom to form a little "hill" (you can add some slow release fertilizer at this time) place the plant on top of this and spread the roots out in all directions. Make sure to keep the previouse soil level line on the plant even with the current soil level (don't plant too deep or too shallow)
Start filling in with soil, tamping down when it is about half full, and filling the hole up. Make sure you tamp it down really well, as air pockets in the soil can actually kill the plant. I usually pack it down with my feet. Water thoroughly.
