In the playoffs, yes. There has to be a winner. During the regular season, originally there was no overtime. It would be a tie game. Then the NHL added a 5 minute overtime. Teams would play 5X5. If teams were still tied, it would be a tie. The 5 minute 4X4 OT followed by the shoot-out was only added recently. There is no shootout during the playoffs.

in the playoffs, there is sudden death overtime. That is simply because in the playoff games there must be a winner to determine which team would advance in the next round.

In the regular season, after a period in ot, and if noone scored the game can end in a tie.

It is now, but it used to be just another full 20 minute period, so that's how you get 5-3 scores in some old overtime games, but it's been sudden death for a while now I think.
