What House says about the leg problem:
A patient: "What made you a cripple?"
House: "I had an infarction."
The patient: "A heart attack?"
House: "It's what happens when the blood flow is obstructed. If it's in the heart it's a heart attack. If it's in the lungs it's a pulmonary embolism. If it's in the brain it's a stroke. I had it in my thigh muscles."
The patient: "Wasn't there something they could do?"
House: "There was plenty they could do, if they made the right diagnosis, but the only symptom was pain. Not many people get to experience muscle death."
The patient: "Did you think you were dying?"
House: "I hoped I was dying."

Dr House Leg Injury

It has never been fully explained, and this has been done on purpose.

It's common in literature and fiction to have a "plot convenience" injury or disease. This was more common in the 19th Century when female characters would come down with a romanticized "wasting" disease that was loosely based upon Tuberculous.

Keeping the nature of House's leg issue vague accomplishes several goals:

It allows people to project their own injuries, or those of people they love, onto House. The audience is then able to connect with House emotionally, remembering how their own physical injuries impacted them.

No matter what happens in the show, House always has an automatic "underdog" status, which means the audience will root for him even when he's being a royal bastard.

The character flaw of the pill popping is harder for people to challenge. No one is coming out saying "I had injuries worse than House and I'm not an addict." By not explaining the scope of what happened, people are unable to gauge or guess how much pain he might be in. As a result, they are less likely to judge him based on any such assumptions. Even the folks who believe addicts are just weak willed are left on an uncertain footing.

I don't remember exactly what caused it in the timeline before the series started (probably got shot by someone), but there was an episode (last season I think) where the muscles in House's leg were starting to atrophy (die), and they ended up putting House under and removing part of the muscle to ease his pain. For a short time after that, he was able to walk normally again. I'm not 100% sure why he went back to using his cane though.

In a recent episode, House was having dreams about a war that he was supposedly in, and if I remember correctly, they showed him getting injured. I'm not sure if a disease was involved, but I would think that the damage was partially irreperable.

Amazing show, by the ways :)
