If it's before the game starts, the home team checks the weather pattern along with the umpiring crew chief. If the game can't be played to completion, or at least 5 inning to make it a complete game, the decision to cancel is then made. Once the game starts, the umpiring crew chief talks with both managers, to let them know what is going to happen.
Bottom line, the umpires always make the final decision.

During spring training and the regular season, the home team decides whether the game will start if there is a question of weather or field conditions. Once the lineup cards are exchanged with the umpire, the umpire in chief is in charge of the game.
During the playoffs, the commissioner's office decides whether a game will start.

i agree with chelebee. It is the umpires. The managers really don't have anything to do with it. When I was in the minors, my manger went to his car with his mistress. The game got called and i think he was still missing?
