hes obviously been well cared for - lucky hes lived for so long
the only thing you can do is keep toys and things out of the cage, leave him alone and keep him in a quiet place, plenty of bedding to keep him snug he must be in pain poor thing...

OVER FOUR YEARS OLD!
After you've been to the vet you might want to contact the guiness book of world records, I've never heard of a mouse living anywhere near that old!
The baldness and scratching is most likely to be mites. These are more likely to get on a mouse when their immune system is down, like if they're ill at all. The bulging eye would suggest the mouse is ill. The vet can treat the mites, but - and I don't really like to say this - all the hamsters and mice I've had died not long after having a bulged out eye or closed up eye. My first hamster had one eye closed up for ages, and then the other suddenly prolapsed one day.
But then, I've never had a rodent live longer than 3 years! Good luck at the vets!

My guess would me mites.
They affect older mice more than youngsters.Take it to the vet and he can confirm if this is right. If it is an injection should help.

Make sure that he it comfortable, take away any toys/objects that he could hurt himself on. Make sure he has a supply of fresh food and water and he is warm. Leave him in a quiet room to rest untill you take him to the vets. Sorry to hear he isnt too well xxx

Could have skin mites, and that's something they get with old age. Needs vet treatment immediately - it will drive him mad with the itching.
