believe me, honey. people can be ugly and conceited at the same time so the case you said does not always apply.
if people have said this about a certain person, it usually means they don't like the attitude of that person regardless of what they look like.
beauty, wealth, an impressive IQ, or anything that is superlative is never an excuse to give people attitude.
if someone's conceited, they are holding or are characterized by an unduly high opinion of oneself so it does not always spell out one's physical traits.

one might think that one is superior over others if one is "pretty", because, indeed, pretty people do have an advantage. when one does capitalise on it, when one already taking advantage of people, or any unpleasant thing about it... oh, goodness.

bottom line is, though. if people say one is conceited, it usually means that this someone has, indeed, shown their high opinion about oneself. so it's usually bad.
