Yes, protruding scales is a suire sign of dropsy. Dropsy is the symptom of an internal bacterial infection causing organs to function incorrectly and allowing fluid to build up in the fishes body cavity. It cannot be treated with epsom salts or by dietary methods. Those will work for constipation, but not dropsy. The only realistic treatment for dropsy is the antibiotic Furanace or Tetracycline if Furanace isn't available. A long term treatment of 14 days is your best bet, but to be totally honest cure rates for dropsy are around 10% so expect the worst even with prompt treatment.

that does sound like dropsy. dropsy is a secondary condition, and only occurs after the primary condition has affected the fish so much that internal organs have started to shut down.

you can try and make the fish as comfortable as possible. feed cook de-shelled pea to make sure there's no blockages there. and try an epsom salt bath, details on treating that are in the link below.

but honestly, once fish start to show these obvious signs, it's usually too late for a cure.
