No, you do not always know why you committed an act (or omitted to act). This is because introspection (due to the nature of man) is not a perfect tool. Often we do not know why we feel in certain ways, for example, a person against abortion may think that abortion is murder, but they cannot say why they feel this to be true. Just like a person who believes that abortion is about choice cannot say exactly why they hold this belief and not the other.

There are also situations in which we are prone to self-denial, covering up the true motives behind an action because it may upset us (such as during cognitive dissonance).

I usually know why I did something but sometimes I don't make the best decisions. Sometimes my judgement is compromised by other factors (emotional issues, lack of sleep, feeling stressed, tired, sick, run down). In a cloudy state of mind I may do some wonky things that I'll regret later. I've never done anything too terrible I don't think...thankfully...I think the point is that often we're on auto-pilot. We're so busy & have so much on our minds, so much to do that we're often acting without thinking. What was I thinking when I did that? I wasn't thinking! That's why I did it!

Yes always!
Because I do something only After deciding ! This is due to the fact that as Thoughts come we tend to identify with them. the intellect then takes a decision. then we perform the act. next we are waiting for the result. we expect it To be what we intended it to be. If it is as we expected then we are happy we get some satisfaction, if not then we will try to change some thing to make it the way we expect it. This goes on till the desired result is got.
Most of the time we may not be able to get what we want so we will keep thinking about it. This is just the natural way.

In order to be happy ,What we really need to do is not to identify with our thoughts. Then we will attain a state of tranquility which is different from the daily routine, this way we will be able to come to a state of meditation,all the stress of our waking state will be reduced and the true meaning of life will be revealed to us!

I personally always know why I did something. Sometimes its in the heat of the moment. Many times I've opened my mouth out too loud and said hurtful things during an argument. But I know I will always come back and apologise when I've calmed down. That is a must for me.

No, I don't always know why I did something. The problem is that sometimes I just don't think before I act or I act before I've thought it through all the way.

depends on the state of mind while in the act of committing. if you are intoxicated with alcohol or drugs you may care less about doing something that you would normally never do, then regret. But I truly think that you always know what your doing, when and why.

Most of the hours of the day I have know clue why I did what I did, esp. when I'm at work, I try to detach from reality.

Purposeless actions could either be reflex actions or merely to pass the time, which no one can avoid totally.
