what is the theory of how planets form

German astronomer Johannes Kepler deduced three laws (Kepler’s laws) that describe the motion of the planets. Examine the chart and tell what his calculations tell us.

Check this out it doesnt actually answer your question but it might interest you.
according to Einsteins theory if mass travel's at the speed of light it turns inti energy and if that energy slows down it will become mass again, So if you think about it if you believe in the big bang theory massive amounts of energy just flew out everywhere at tremendous speeds what if that energy turned into matter after it slowed down and turned into mass or rocks and crap and every massive rock would have gravity and pull more in and maybe some of that energy stayed in some of the rocks and that could be the stars.
sorry that doesn't answer your question but sometimes one question or theory just leads to another so I thought I would share mine

Kepler used a large amount of data that had been collected by a Dane, Tycho Brahe, which involved very accurate observations on the position of planets on different dates. He developed his 3 laws by empirical means, using proposed models and fitting the data to them. The 3 laws are briefly:
Planets revolve around the sun in elliptical orbits.
Planets sweep equal amounts of area in similar time intervals
The distance from the sun and period of revolution are related by a 2/3 power relation.
This is the first chart in your web page.

His results were verified about 75 years later by Sir Issac Newton who demonstrated that their results were derivable from his theory rather than exhaustive data crunching.
