Please don't include Fisherman's Wharf or Alcatraz (he's seen them). Preferable options that don't involve too much walking (he's injured his foot). Thanks!

Take him to the ferry building, great food, amazing views.

Columbus Ave between Jackson and Union, lots of places to stop and rest, great restaurants, and coffee shops, Washington Square Park, lots of folks to see.

The Japanese Tea Garden in Golden Gate Park. Very small and you can have tea in the garden.

Yerba Buena Gardens and the Museum of Modern Art. A lovely green space right down town with a museum near. Lots of places to sit. I think MOMA is 1/2 price on Wednesdays after 6 pm. Check their website to be sure.

How about a drive down Highway 1 by the Cliff house (stop in if interesting) some great views all the way to Santa Cruz. Drive along West Cliff drive in Santa Cruz and stop at the Lighthouse to see the surfers. (only a few feet from parking to the lookout) then continue driving for about 1 mile to the Santa Cruz wharf ( a 1/2 mile wharf you drive on and can stop anywhere for a bite to eat or things to buy or for viewing seals or sea-otters. Then drive by the boardwalk (or stop and walk thru) Keep going along the coast as you leave the boardwalk. Stay overnight somewhere in that area and there are plenty of good restaurants in the area.
You are about 2 hours south of San Francisco if you use the freeways instead of highway 1 on your way back.

I would drive out Ocean Beach to Skyline, to Hwy 1 over Devil's slide to Princeton for a leisurly lunch on the coast. T

The reason why: In a few months, you will no longer be able to travel at edge of Devil's slide because a by-pass will be completed. You might drive on down the coast and stay over night in Santa Cruz or Carmel (how far, depends how much time you have -- make it leisurely). Sit on the beach, go out for dinner, then drive back the next day.

Take him to visit some relatives, if you have any in the area. Or go hang out or eat with some friends he might enjoy meeting.

Why don't you go to Monterey and Carmel. You can drive around the area. Its very scenic.

You can also go to Sausalito or Muir Woods. The boat rides in San Francisco are also enjoyable.
