If it is a gal, she asked you for a reason - probably suspects she is not quite at her best. Tell her the truth gently, with a suggestion if possible. Remember, this is your BEST friend. For example: "You're OK, but not quite your usual lovely self. Did you get enough sleep last night? No? Well, a little make-up should work wonders for today. Would you like for me to brush out your hair?"
If it's a guy, just laugh and say, "No! You look like you were pulled through a rat hole backwards! What happened?"

Well I wouldn't be brutal about it, but I would definitely tell them, cause if you dont then someone will, and they might start to wonder what kind of friend you are if you didn't tell them the truth

Usually my friends tell me the truth when I look bad and usually it's when I don't feel good so I'd be honest and say you look like crap! What's wrong? Are you sick?

I would nicely tell them the truth, or most of it. I would ask them what was wrong and suggest that they just look "out of sorts" or "under the weather".
