The major risk with a monroe (or other oral piercings) is tooth and gum ersosion. Proper placement and jewelry can minimize the risks, but not eliminate them.

There is always the risk of infection with piercings, but proper aftercare and a licenced piercer will minimize that risk as well.

You will experience swelling for the first week or so and tenderness of the piercing site for up to a few weeks.

Since it is a lip piercing the inside of the piercing can rub on gums and erode them causing pain. Also plaque from eating and sleeping and just daily wear and tear can grow on the metal and infect the piercing. Some of my friends who have one, when they take it out for work have a small hole that's there, and other friends didn't want it after wards and it left a scar. You have to be very diligent in taking care of a Monroe piercing since it is in the mouth and mouths tend to house lots of bacteria.
