depends
lets say you pay for your prescription at Walgreen's by credit card, but you buy some other items while there,, food, drinks,, candy,, gum,, whatever. How does the IRS (if you are audited) know what you bought?
Or if you buy items at Lowes or Home Depot for your business,, but buy a new gas grill for yourself on the same credit card bill.
If you pay a doctor or hospital,, that's a good bet,, some may not be so clear if the feds take a look.
