I don't believe that could be legally binding anyway, but no I would not sign that form.
Unfortunately for many doctors today, the cost of malpractice insurance is so high that these are some of the things they feel they have to do to try and minimize cost.

Do you know why it's so hard to sue for malpractice? Most doctors have shingles out that say
Dr. ???
"Practicing Physician"
you can't hardly sue for somebody practicing.

Sure, since malpractice is a professional negligence claim and you can't contract out of negligence, the form would have no effect on that cause of action.

Why would anyone in their right mind do that ? Basically you are telling them it's O.K. for them to do whatever the hell they want!!! If a doctor did that to me I would change doctors.Check to see whether or not that doctor had been sued for malpractice.Ask a lot of questions!!!

No, that's why my doctor dropped me as his patient. He's not doing a test on me without taking responsibility. I haven't seen him walk on water yet.
