It's leaning on its side in the yard and I was amazed to see it was still alive. There are some roots still in the ground but a lot are above ground. Is this something I can save?

Yes, by all means. You can dug where the uprooted roots came from and then gently reposition the whole plant upright but be very careful not to damage the remaining roots in the ground. The exposed one would be dry now but that doesn't mean they're dead. Now is the right time to put some future support and also some good compost.

Your rose should be fine. Good luck.

You can also put in deep stakes to tie the tree down so the next big wind doesn't up root it again before it has time to reestablish itself.
