In Pennsylvania, the title companies are required to search support liens as a part of the title work required when a person needs a title search in order ot sell a property or get a mortgage.

Check the laws of the state of residence of the spouse.

We do have a Uniform Reciprical Enforcement of Support Act in all 50 states. http://www.law.upenn.edu/bll/ulc/fnact99/1920_69/ruresa68.htm

Here in the Uk if a court orders a payment and it isn't paid you go back to the court and advise them The other party is in contempt of court for failing to pay and the court can issue a warrant for his arrest.
It may be the same on your side of the duck pond

A Lien by the way is on property. example a garage can hold your car if they have worked on it until such time as their bill is paid
