That would really depend on where you are from and what company you are using. Here in Canada (and probably in the USA).. broadband accounts are usually billed monthly.. either through a cable company or phone company.. and you generally have 'unlimited' time on the account.. You can use it whenever you want.
Many companies will have a 'cap' or 'limit' on the number of megabytes that you can download... But it's completely up to each company what their limits are. I know some are say you can download up to 70 Gigabytes per month... others.. higher.. others .. lower... And even if you do go over, most companies will just let you know.. and if it keeps happening, will then ask you to switch to a different type of account. (the average 'web surfer' won't use that though.. it's people that download huge amounts of data that will use it faster)..
I would call up your local cable and phone companies (or look at their websites) for their current pricing.

Mine (and I expect all) charges a monthly fee. If they charged by the minute I'd be in trouble since I work on the internet.

Depends upon the billing plan you have subscribed too, if you choose unlimited usage plan then youll not be charged

You are charged monthly or by a contract. Dial-up is charged by how many minutes you are on.
I may not be 100% correct though.

in broadband ur charged for the data u uploaded or downloaded some operatores don't charge anything for uploading but they charge for downloading.

In my location, it is unlimited usage and billing is fixed amount monthly. Service provider just controls bandwidth.
