I think it's with the political system. The king with an Empire and the president or prime minister in a republic. But Voltaire remarked that the Roman Empire was it wasn't holy nor Roman nor Empire because of the faults of the empire. Rome as a republic is better right now than before.

The main difference is that the Roman Republic was a republican state (not to be confused with red state, lol) whereas the Roman empire used dictators.
The Roman Republic came first and through civil wars it was weakened politically. Julius Caesar is considered the first Roman dictator.

Not much to the Romans. The state was still the "res publica" (public thing) before and after Caesar crossed the Rubicon. "Imperator" simply meant a victorious general. Julius Caesar was unpopular for taking the title "Dictator for Life" and many were afraid he would be a king. Augustus Caesar never took the title "Rex" (king) but called himself "princeps" instead. Romans still had their citizens' rights.

When it was a republic competent men competed for leadership. After Augustus stupid, insane, and incompetent emperors could control the government as long as the had the support of soldiers. Later rival generals lead their armies into battle for control.
