Not at home, but I used to work in a glass-sided office building. Birds would whomp into it pretty much every day.
I would go outside, after work, and some days I would find (I am NOT making this up) as many as eight birds, dead or unconscious, in the shrubbery alongside the building. The janitor had a once-a-week routine of removing the assorted bird corpses.
In my old house I used to hear a weird clicking noise outside our dining room window. It sounded almost like a cricket chirp, but it was different and unfamiliar. It turns out, it was a hummingbird, and he was "fighting" with his own reflection in the window! The clicking was his little beak, trying to poke out the eyes of his "opponent".

Yes, twice that I can remember. The first time the bird died instantly and the second time I saw that that a bird was still alive and stayed with it to watch over it but it eventually died too. I buried them both. It was sad.

yes twice and they were in days of each other
one hit the drivers side of my car while i was driving scared the crap out of me and the other was the front window of my house all though it didn't die it just hung around my house for a few days i guess it was hurt and did give it food and water and then it left or a cat got a hold of it

No, well sort of. We have a duck couple that walks around outside my store where I work and the girl was standing but the open glass door the other day and this woman scared her and she flew up and into the door it was sad = (( they havent come back since that happened.. stupid woman.

No, but while I was driving I hit a bird that was flying in front of my car by accident and he got killed. He landed on my hood and no I didn't hold any services for him as i didn't know his friends to contact.

definite. I additionally hit a livestock egret with a unload truck on a similar time as traveling at sixty miles an hour. It hit on the staggering edge of the windshield, and it crapped throughout each thing on result!
