Depends on how into your date you are. If he/she is someone you really like, leave the phone turned off. If it's a blind date, or someone you don't care for, say excuse me, answer the phone, and go on from there.
I would consider it very poor taste, and a qualifier for a "no thanks" on a second-date request if someone I was with found me so uninteresting they had to talk to their friends on the phone.
Emergency phone calls, of course, trump anything else. If it's the babysitter, etc. of course you answer.

I don't think so. Only answer your phone, if you must, to say you can't talk now and will call back later, period. Then, DISCONNECT.
I'd say turn off your phone on a date (you have to -- or should -- in the movies or at the theater or other event). That's why you have texting and an answering service.
Why, you ask. Because it is, in a word: rude.

Only if it's a real emergency like work calling or a family member that has called previously a few times. Otherwise, let it go to voicemail and check it when you go to the bathroom.
