It's hard to explain without science termonology.
TAbisically the body has decided the sex of the person, but the persons mind has gone the other way,
(men feel more like women in their mind, and women feel like men in theri mind)
Some don't go as far to get the operatiopn and some do.
But I remember watching a CSI episode witht he transgndered community, and at the end Grissom told his tran friend ( who was just feeling sad at hate crimes)
When we started out as single as organisms, we had teh oportunity to change gender ( like the clown fish, or most animals actually can do that)
so to be born just ONE gender is the mutation.
There is nothing wrong with it, They want acceptance as much as anyone, and the first step is to accept yourself.

Have you ever heard(of course the guy saying it usually is drunk and horny, yet I digress) a guy say "I'm a Lesbian trapped in a man's body!"? Well, that's basically what it means. See, a person(male or female) CAN truly feel as if they are the wrong gender and cannot tolerate their body type and cannot identify with their bodies gender at all. This is where the term "transgendered" starts to come into light. The next step for these people is usually a sex-change operation. Along with some gender counseling to help them more readily identify their new gender as well as hormone therapy, they have good if not better lives afterward. That's basically what "transgendered" means. Although I could be wrong, because I have been wrong in the past you know.

Transgender is a term coined around 1971 by a cross dresser named Virginia Prince (her female persona name).
It does not refer to transsexuals or those who feel they were born in the wrong body, although some transsexuals use the term because it sounds like it should refer to them.
I think her rationale for the term was that cross dressers have one sex (that they do not deny) and like to dress like the opposite sex to look like the other gender, but have no intention of being that gender. So they temporarily cross conventional gender expression boundaries and ultmately return to their own real gender appearance and dress.
