Back in the times of the old west, the revolver itself had to be ****** before the triger could be pulled. A person had to manually pull back the hammer; once it was ******, the trigger could be pulled releasing the hammer and firing the bullet.
Modern revolvers do both actions using the trigger. You pull the trigger and the hammer gets pulled back and then released as one single action. Of course you can still manually cock the hammer back before pulling the trigger, but not sure it would be any faster than simply pulling the trigger multiple times.

Old revolvers used to have the entire cylinder drop out. A gunslinger kept backup cylinders with bullets already in them and swapped them out as needed. The bullet extractor designs came out later.
