Yes they can. Happened to me (naturally cursed with 38J's), I had serious back pain and health issues related to big breasts, got a reduction (to a 38C),and now I'm feeling great.
With fake boobs, you all of the sudden POOF have them on your chest, so it won't be long before they start having back pain too, unless you didn't go too big (up to a C is good, over that is asking for back pain).

Hi, I wanted to tell you about my experience with the CurvyBust cream. I'm 24 years old and I've spent pretty much my whole life wishing I had slightly larger breasts. I'm normally a size A, I'd love to be a size B, especially when I wear certain kinds of low cut tops. My breasts are standing out more, and they are fuller and rounder. Sometimes I am able to go out without a bra, now I can't. And I have a feeling that I might have to actually buy new bras if the growth continues. I'm looking forward to seeing how the next month goes. There might actually be a size B in my future! Woohoo!

Large breasts can put a strain on the back reguardless if they are real or enhanced. A large boob is a heavy boob no matter if it's filled with fat or silicone.

Yes they can. Large breasts put a strain on your neck and upper back muscles sometimes enough to cause chronic pain.
It is one major reason for breast reduction surgery.

Indeed they can and do on countless people. The spine already handles a load consisting of the central nervous system. vertebrae dont need excess pressure. nor does the lumbar region and the added weight of excessively sized breasts wether natural or artificial can result in pain in the spinal region. donalore_43

Naturally large breasts or surgically augemented breasts can both cause back pain. Breast tissue (or saline pouches) can weigh quite a bit putting strain on both your back and chest muscles.
