You should insulate all the perimeter walls in the crawl space. Is the ductwork from your furnace located in the crawl? If so, do not add insulatin to the 'ceiling' area. Any heat from your ductwork will rise and help keep the floor warm.
When you insulate the exterior walls (from the inside of the crawl) make sure you do not block any vent. they are necessary.

I would recommend insulating the floor. If you use insulation with the vapor barrier attached remember the vapor barrier goes towards the warm side, or up in this application. They make stiff wire that you place between the floor joists to hold up the insulation so you won't have to try to staple it in place.

No, but I would recommend insulating the floors above. They have some crafted faced insulation bats that will staple in the floor joists to keep the floor warmer in winter.

I'd go w/ Fordman on this one but go into crawl space during daylite hours and see if you can see daylite in any cracks. Fill them in if you do, also close any air vents in cold weather
