well the formula for that is y=mx+b where m is your slope and b is your y-intercept so..

y=x+4

the general equation for a line is y = mx +b
plug in what you're give and you should get
y = 1x +4 which is also the same as y = x + 4

The slope- intercept formula is
y=mx+b

Plug in the numbers and you have

y=1*x+4
or y = x + 4
