Contact your local agency. You will have to fill out a bunch of forms and they will ask for reasons why you think you need disability.
They will schedule you an appointment to see a doctor and yes the doctor will help with the decision on whether you can claim disability.

Here in the US you will have to have medical proof that you are disabled. Typically, your claim will be rejected the first time. The Social Security Administration gets a lot of claims from people who just want an easy way to make money, so those people have ruined it for the honest and legitimate claims.

Yes, your doctor has to go through the process with you. More then one doctor is often needed. You will have to see their doctor as well. And there is a lot of paper work. It is very common to be refused the first time and it is better to have a lawyer. It took me two attempts. You have to not work for 6 consecutive months before applying,

You cannot declare yourself disabled. A doctor has to start the process. After that you may have other obsticles to pass. If insurance is involved ll have to see their doctor too. If you are trying to collece social security you will have to pass all of their tests too. The process can take up to two years or so.

Yes, you have to see a doctor. Depending on the disability, you may have to see the state's doctor(s). Go to your local welfare office (Human Services) and ask to see a caseworker. There will be forms to fill out.
Good luck. :)

Yes a doctor has to state you are disabled and then you have to see another doctor (FROM SSI) and they have to agree with your doctor.
