Narcissism is a personality disorder. Typically, people with personality disorders are less aware of their condition than people with mental disorders. Mental disorders are also more likely to change over the course of time, whereas the assumption with personality disorders is that they'll always be a part of a person's functioning.

Someone with narcissistic personality disorder often needs to undergo some sort of very difficult life change brought about by their disorder before they can take a step back and think critically about their behaviors. There is hope, but it can be a pretty long-term and difficult thing to manage.

I dont believe they do recognize their disorder, but they do know they are different..I dont feel confronting them would do any good what so ever because they dont believe there is a problem at all..if there is a problem..its someone else..and they are the special and gifted one..I have read that..when they go into their raging fits..to throw stuff right back at them..just like they do to you, it will back them down temporarily..but I dont think they can change..and most recommend walking away..good luck..i know its hard to deal with this sort of person..

No they are not aware of their disorder because it's just a personality to them. They know they are different but they find that an asset and it assures them of their greatness or beauty.
You cannot change them.
