If the road to hell is paved with good intentions, then the slide into hell is made by a complete lack of them. When we have no intention in our mind, no purpose in view, no reason in us being this way or that way, of doing this thing instead of that then we can see that we are getting nowhere, only aimlessly drifting along. This then is for sure that we must have intentions, now whether they are good or bad we can check and see.
The saying is famous because of the irony its hides into its deep structure, which pinpoints an important aspect of human nature: need for moral justification is most common to human nature that we need to feel morally just for whatever we do the thing we do, and that without sufficient moral backings, we would not be able to proceed along doing anything at all. This is human nature. We need reasons to act, and then we need these reasons to be just. Especially, when it comes to facing the consequence of our actions, we are often never sure if we are really right. For this purpose people often resort to religion, socially approvable moral purposes, and concepts of goodness and or goodwill of common people to seek justification for what we plan to do. And at times some people go a little too far into this, they somehow assure themselves and the people around them that what they are going to do in right thing and that they have approbation of gods on their side.
In contrast to this are essential human traits that are also in human nature. Human mind is endowed with high qualities that make a person conscious of the importance of his or her position in life. People when take responsibilities of their actions and therefore deliberate upon the widest range of possibilities of succeeding or failing then they find the very justification form within. We all intend to do good things in this world but how much good we are able to actually do depends entirely upon what type of people we are. But if we a subjective to a concepts of goodness outside our being and try to borrow goodness for intentions that are entirely our own then we often pave our paths along wrong direction. The goodness is inside and so are our intentions, two things in one purpose combined.

The bottom line is that you must do things with sincerity, the right intent. Lots of people do things with hooks attached, or selfish motives, or by expecting things in return. Sometimes it's very subtle, and they do things to create a greater need for themselves, like a slave master relationship (the government bureaucrats are like this). So one must have true intentions and not try to get something out of the altruism, otherwise the slightest selfishness can ruin a good intent.

The road to hell is paved with bad intentions too. The phrase just shows the wisdom of realizing you can't control outcomes.
Do something when you feel strongly about it, when your own conscience will convict you of your inactions, regardless of possible outcomes. In other words, if your inaction will bother you more than any possible result of your action, then act.
