It really depends on what you are doing.

It could be that you made a human error - took a reading wrong, read an instrument wrong, etc.

It could be a problem with your equipment (balance not calibrated correctly, pH meter not calibrated, etc.)

It could be impurities. You'd end up with a mass that is artifically high.

It could be loss of product.

It could be an incomplete reaction.

You could have wet equipment that diluted your solutions.

There are many reasons why observed and accepted values can differ, but they depend on what you are doing in the lab.
