when a cerificate holder reports that her certificate is missing, CGNS does not re-issue a duplicate cerificate. However, we can send you an official letter verifying the date of your certification. to oreder this service, write to CGFNS stating that your request and include your CGFNS ID Number, your birth date, and documentation of any name change since you received your cerificate. It is also important that you sign the letter. Enclose a the appropriate fee, address the envelope to CGFNS and mark it
