There are still Kings..An Emporor is just a ruler of Kingdoms..Germany's last Kaiser, Wilhelm II, was not only Emporer of Germany, he wa also the King of Prussia. Countries could be run by Counts, Dukes, Barons, Kings, Ect . This stuff is NOT just from the Middle Ages. The Queen Mother was the last Emperess of India.

It would actually depend on the country's type of government. Parliamentary governments have a Prime Minister and Republics have Presidents.
btw, the whole emperor and king definitions are so old they only apply during the middle ages or long before that.
