I would simply say - "I hereby give you notice of my intention to terminate my employment with your firm, with effect from today's date. Therefore my last day of employment will be ....." And then sign off. I would keep it very short and very simple. When you hand the letter to your boss, then you can tell him face to face why you are leaving which is better than putting it in a letter. Also it shows that he doesn't frighten you cos of how he's been and will show that you're confident and what a loss to his company you'll be. Good luck.

When I left my job about 3 months ago - I thought long and hard about this, as on one hand I really wanted to tell me boss exactly what I thought and on the other I wanted a good recommendation from him eventually I went with the following advice:
1) Be brief
2) Be pleasant
3) Don't say all the things you really want to say
Dear....
As required by my contract of employment, please consider this my notice that I will be resigning from my position as....
My last day will be the ....
I wish you every success in the future and would like to thank you for having me as prt of your team.
Kindest Regards.....
A company can not give you a bad reference, but they can refuse to give you one - which would say it all really. Don't leave things badly, even if you can not wait to get out of there - it is best to be the bigger person and leave nicely.

Revenge is a dish best served cold. Just write an general letter of resignation and leave it at that. It's not worth the hassle otherwise

Your resignation date is the date you turn in the letter. You can turn it in two weeks early giving notice that you want date certain to be your last day, and that is the date your resignation is effective.

Be as polite as possible. In the long term there is no advantage to being rude other that self satisfaction.
By law you cannot be given a damaging reference, but your employer might make it curt and unhelpful to your next employer.

Keep it short and simple stating that you are resigning and how much notice you are giving. Also ask for them to confirm your finish date.
You can give a reason if you wish but again keep it vague.

1 tip....never bad mouth your previous employer....i could have done for numerous reasons....but refused to as the business community is a very small world.
do it short and to the point.
