It means Regina V blah. in other words Her majesty the queen V Blah. It is the queens council V blah. All Judges in high Court are acting as a representative of the Queen.The queen signs all documents as Elizabeth R. The letter R is for Regina.

Here in the U.S., a criminal case is usually People v. Blah, which is short for People of the State of ______ v. Blah.

It depends how old the case is. Between 1901 and early 1952 it stood for "Rex" -- the Latin word for king. Since then it has stood for "Regina" -- the Latin word for queen. That is why, on the whole, it is a safer bet to say "the Crown".
