Here's a good one. The two characters are 'actors.' They are rehearsing a scene in which one of them is holding a gun. The actor holding the gun is a female. Another woman is in love with the male actor (who is in fact one of the perons in the argument/fight). This other woman knew that an argument would ensue and placed a real bullet in the gun. When the scene actually takes place, the female actor fires the weapon and actually kills the other man.
Prior to the scene, someone actually heard the two of them arguing. This could also be a great sub-plot.

Well, you could have them fight a while, get tired, decide to talk through the problem, and then have one collapse from brain injuries or internal unjuries immediately after, so one dies anyway after the problem is resolved. It happens but is still ironic enough NOT to see it coming. Especially if the characters are getting along, it would throw of the tone or create a dramatic change in mood.

lots of choices ...
one leaves mad or hurt and gets into a car, and then loses control because their emotions are so wild, and they are killed
one accidentally falls or stumbles and is killed
one accidentally stabs or hits the other, killing them
it all depends on the mood and tone of your story, do you want it to be accidental or tragic, what was deserved by the character or a horrible tragedy or irony. That choice is yours, but the story must be believable, and you have to write it so it makes sense, based on the characters and the conflict.

They could be fighting near a large pane of glass, and knock someone who is in the room with them into through the glass and be impailed.

But you'd have to read the chapter to understand that she had no choice in the matter.

I don't know how they should die, but it should be ironic. For example, if they are on the swim team, they should drown in some way.
