depends on the day, but i dont usually put ketchup on my fries, dip them in a chocolate shake it is way better

I put it in a pile next to (but not directly touching) the fries. If a fry happens to get trapped in the ketchup pile I consider it a casualty if it's in there for more than 30 seconds (it gets too soggy). I only squirt ketchup on my fries if I'm at an event, like a fair or something, and they give you one of those cups of fries. But those are "event fries" so you can get away with it.

Depends if I only have ketchup. I used to squirt hot sauce, ketchup and ranch dressing on my fries, but most of the time I don't even use ketchup

Most of the team when I lived i n England I slobbered it all over them but olver here in the US I tend to leave a pile beside the fries.
